HIPAA and security

HIPAA and security readiness review

A written review of access, logging, encryption, backups, vendors, and BAAs. You get a gap list with owners before the next hospital security questionnaire.

Who it's for

For the team that has to answer the questionnaire.

Teams about to answer a hospital or payer security questionnaire

Founders who store PHI and have no written gap list

Companies that can find some BAAs and not the rest

Product teams connecting to an EHR

What we hear

The questionnaire is more specific than the slide deck.

"The questionnaire asks about audit logs. I do not know if we have them."

Founder, digital health

"Three vendors touch PHI. I can find one BAA."

COO, health SaaS

"We encrypt the database. I cannot say the same for the backups."

Head of engineering, clinical product

"They asked how EHR access works. We have a slide, not a control."

Founder, digital health

This review turns those blanks into a list with an owner on each line.

What we do

What you keep at the end of the review.

The review

Three steps. A list you keep.

This is a readiness review, not a certification and not an audit.

Step 101

Scope

NDA first, then read-only access to the code, the cloud account, and the vendor list.

  • Systems in scope, including EHR connections
  • Where PHI is stored and who can open it
  • The questionnaire, if you already have it
Step 202

Read

We read access, logging, encryption, backups, vendors, and BAAs. We do not take a copy of patient records.

  • Access paths into production and the EHR
  • Logging, encryption, and backups
  • Which vendors have a BAA and which do not
Step 303

Write

You get a gap list. Each gap names an owner on your team.

  • Gaps in the order we would close them
  • What you can already answer
  • What you should not put on the questionnaire yet
NDA before accessSigned before anyone sees a repository or environment.
Read-only by defaultWe review configuration. We do not take a copy of patient records.
You keep the listThe gap list is yours, whether or not you keep us.
Not a certificationWe do not certify HIPAA compliance. We are not your auditor.

CareMetxHealthcare

A platform where health information moves with the case

CareMetx helps patients get on and stay on specialty therapies. The work includes benefits verification, prior authorization support, and patient support. Patients, providers, pharmacies, payers, and manufacturers touch the same case, and health information has to be handled at every step. Maxiom's role was staff augmentation: architects, engineers, developers, and designers joined the CareMetx team on CareMetx Connect, on Azure and Microsoft Dynamics, in a service oriented architecture. That engagement was not a HIPAA audit. We do not certify HIPAA compliance.

FAQ

Straight answers.

Is this a HIPAA certification?

No. We do not certify HIPAA compliance. We are not auditors. You get a written readiness list with owners.

Will this make us pass a hospital review?

No. The hospital, the payer, or your auditor makes that call. The review tells you what you can answer and what is still a gap.

What do you look at?

Access, logging, encryption, backups, EHR connections, vendors, and BAAs. We do this with read-only access after an NDA. We do not take a copy of patient records.

What do we keep?

The gap list, the vendor and BAA inventory, and the notes on which questionnaire answers the list supports.

How is this different from a fractional CTO?

This review ends with the list. A fractional CTO stays and owns the architecture, PHI, and vendor calls after that. That offer is at https://cto20.com/healthcare-cto.

How do we start?

Book a time. It is a 30-minute call about your product, your vendors, and the questionnaire in front of you. You get a clear yes or no on fit.

Bring the questionnaire, or the product.

Your stack, your vendors, and the questions you cannot answer yet. You leave with a clear yes or no.
